My health equity-related work stems from my employment with HealthLinc, Inc. HealthLinc is a Federally Qualified Health Center in Northwest Indiana. By following a Patient-Centered Medical Home model, HealthLinc can provide care that is coordinated, comprehensive and accessible in order to achieve improved health care outcomes and satisfaction for patients and their families.I started as the only pharmacist with HealthLinc in 2015 working with patients with multiple chronic conditions. Due to our positive impact on patient care and quality metrics, our pharmacy team has since expanded to eight pharmacists working inside our clinics and three pharmacists working inside our on location pharmacies.
